Chronic Diseases
Helping You Best Manage Your Health
Most of us experience complex health complaints throughout our lives, especially as we age. The good news is many conditions are manageable.
Chronic conditions are those that a visit or two to your doctor won’t fix.
Doctors at our clinic have extensive experience treating patients for:
- Alzheimer’s disease and dementia
- Arthritis
- Asthma
- Cancers
- Cardiovascular disease
- Diabetes (Type 1 & 2)
- Iron deficiency
- Strokes
If you’re eligible, your GP can organise a Chronic Disease Management Plan – also known as a GP Management Plan. These Medicare-subsidised plans guide treatment for chronic and complex medical conditions that have been present for over six months.
We can help you access the proper treatment for your or your loved one’s complex health needs. Please call us on 8579 6838 for an appointment or book online.
